MEDICAL SOCIAL WORKERS

Steps Homecare Services offers professional Medical Social Workers who provide in-home support to patients and their families by assessing individualized needs and ensuring access to essential information, care, and community resources.

They play a vital role in reducing hospital readmissions by helping patients remain safely at home while addressing both immediate and long-term goals. Through education, advocacy, and ongoing support, Medical Social Workers empower patients and families to make informed decisions and maintain health, stability, and independence within their home and community.

SKILLED NURSING CARE

Steps Homecare Services has a team of carefully selected Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) that have an unwavering commitment to excellence and are dedicated to a patient-centered approach collaborating with the patient, their loved ones and physicians to develop an individualized plan of care to meet their health goals.

 Steps Homecare Skilled Nurses provide:

  • Comprehensive assessment and instruction

  • Infusion Therapy/ IV Administration

REHABILITATIVE THERAPIES

Steps Homecare Services offers rehabilitative therapies in the home to speed recovery from illness, injury or surgery. Home care rehabilitation allows you to regain productivity and develop independence with daily activities in a convenient and comfortable setting. Families are also able to be closely involved in the rehabilitation process. Included therapies are
